The modern familiar form is noted as dating from 1913, when many people, especially in the lower classes, had limited access to fresh food, and scurvy was not uncommon. Thus in the original form, eating an apple before bed (especially the harder, old fashioned types of apples) may have helped in reducing plaque and keeping teeth in good condition, thereby reducing the need for extractions. The aphorisms, either in the initial Welsh form or more recent version come from the era when the modern professions had not fully formed, and there was limited distinction between the role of physician, apothecary and dentist. The authors of this article may have been led into error by forgetting their medical history. The truth is that some health conditions cannot be influenced by dietary behavior. A truly randomized controlled trial is the best, most scientifically rigorous way to test this. In order to properly analyze whether eating apples on a regular basis has an impact on physician visits researchers need to first establish the frequency of apple consumption and second, the frequency and reason for physician visits. A strained shoulder has nothing to do with dietary behavior, rather physical activity.The data being used in this study is simply not rich enough to make proper conclusions. There are many more reasons to visit the hospital or see a physician that have nothing to do with dietary behavior than in relation to it. We cannot know for certain why the participants were seen by a physician in the past year. I again posit that this measure makes large assumptions about participants that cannot be deduced from the available data. Furthermore, there are several issues around the physician visit measures. This research makes large assumptions regarding a person’s overall dietary behavior based on the behavior in the previous 24 hours. ![]() Consuming an apple in the past 24 hours should be taken at face value because no pattern of regular consumption can be established based on that measure alone. Additionally, using the previous 24 hour recall period is an inaccurate measure on whether someone that consumed an apple should be defined as an “apple eater”. The first concern I have that the reporting consumption of a single apple in the previous 24 hours does not constitute an “apple a day”.
